Visit Notes & Chart Preparation
The chart work wrapped around every visit: the note itself, preparing for the patient, hunting through outside records, and clearing the inbox.
What the software can do
- Listen to the visit, with consent, and draft a structured, specialty-aware note in the EHR for the clinician to edit and sign.
- Prepare the pre-visit picture: pull the chart, outside records, and claims data into a short summary of what matters for this appointment.
- Retrieve, index, and summarize outside records instead of leaving them as 300-page PDFs.
- Draft patient instructions, letters, and forms from what happened in the visit.
- Triage the EHR inbox and draft routine replies for review.
What stays human
Clinicians review, correct, and sign every note — the census found no documentation product operating beyond exception-based drafting. The after-hours relief is real and well documented. Whether it becomes practice money is a separate question: the answer depends on what the recovered time turns into — capacity, staffing, or simply a saner evening — and that part is the practice’s decision, not the software’s.
Automation levels: L0 analytics only · L1 assistant — drafts and suggests · L2 partial — completes meaningful steps · L3 exception-based — completes routine cases and sends exceptions to people · L4 near-autonomous — none validated in this census.
Products shown on this page have the strongest public evidence in the census; every product is in the directory.
Ambient Clinical Documentation · 37 products
The most crowded category in the census. Technical maturity is high and after-hours documentation relief is well documented; the financial return depends on whether recovered time becomes capacity or staffing change.

Before you buy
- How much editing does a note need in your specialty? Ask for edit-rate or time-per-note data, not a demo.
- Does it write into your EHR’s templates and sections, or paste one block of text?
- How does it handle accents, interruptions, multiple speakers, and telehealth visits?
- Where does the audio go, who can access it, how long is it kept, and is a BAA standard?
- What will you do with the recovered time — and will the vendor help you measure it?
